Sometimes, when a person is discharged from a hospital or other facility, they need a little extra help to get back home.  East Tennessee Personal Care Service has a service specifically designed to meet this need.
 
Transportation Back Home:
Coordinate with Discharge Planner

Transport Client to Their Home

Get Client Safely into Their Home

25 Miles of Transportation (extra miles is $.42 a mile)
Once Client is Home:
  • Provide Any Immediate Personal Care
  • Get Client Settled In
  • Prepare Initial Meal
  • Companionship
  • Grocery Shop
  • Pick Up Prescriptions
  • Check For Home Safety
  • Laundry, Linens
  • Alert Neighbors
  • Prepare Home With Lights, HVAC, etc.
Service includes up to 5 hours of personal care. If the client decides to use ETPCS for 40 hours or more of personal care service, the "Let Us Take You Home" fee will be waived.
